We are responsible for providing recommended or required immunizations to all children birth through 18 years of age and to adult clients. All vaccines for children from birth through 18 years are at no cost, through our VFC program. Parents now have the ability to print your child’s immunization record from our Florida Shot System.
Exemption from required immunizations
Form DH 681 is issued if immunizations are in conflict with the religious tenets and practices of the child’s parent or guardian.
