The Florida Department of Health in Gadsden and the Gadsden County Emergency Services have partnered to implement the Helping Elevate Locally Provided Services (HELPS) program. This program aims to improve health and wellness outcomes for the citizens of Gadsden County through a coordinated system of care.
The HELPS program directs Emergency Medical Services (EMS) responders to refer clients to the Gadsden County Health Department’s Liaison to provide support services and case management after stabilization of an emergency event. Gadsden County residents are offered a personalized plan to address items such as chronic disease management, primary care needs, identification of household hazards, and social support services.
