Onsite Sewage Treatment and Disposal Systems are regulated by the Florida Department of Health and the Department of Environmental Protection under Chapter 64E-6 of the Florida Administrative Code.
(If system has been inspected or installed within the last 3 years, we may have the information on file, or; if you have records of repair or installation, you may be able to eliminate this part of the requirement.) You may be able to locate system information at SepticSearch.
The following forms are required for an Existing System Verification:

- Floor Plan

Application submitted with a Site Evaluation (conducted by certified personnel) - $235.00
Application submitted without a Site Evaluation (to be conducted by Health Department Staff) - $350.00
