Birth Certificates

Florida Department of Health in Gadsden County

Birth Certificates are often needed for entrance to school, voter registration, marriage, passports, veteran benefits, public assistance, social security benefits and obtaining a driver’s license.

Birth Certificates from 1917 to Present date are available locally at the Gadsden County Health Department office of Vital Statistics located at 278 Dr. LaSalle LeFall Drive, Quincy, FL. Death Certificates are also available locally from 2009 to Present date.

Florida birth certificates are issued through the Vital Statistics Office at the Florida Department of Health in Gadsden County. Orders may be placed in person, by mail, or online through VitalChek.

Service hours are from 8:00 am until 4:30 p.m. (Closed for lunch 12:30 - 1:30) Monday through Friday. We are closed on State holidays and weekends.

A valid photo ID (driver's license, state ID card, passport, or military ID) is required with all orders. Birth certificate fees are $15.00 for the first copy; $10.00 for each additional copy. Forms of payment accepted are: cash, check, money order, debit card, Discover, Visa and MasterCard. Please make checks and money orders payable to DOH-Gadsden.

Florida Birth Certificates Can Be Obtained By:

  1. Either parent (if named on the certificate)
  2. Registrant, if over 18
  3. Attorney representing the registrant or parent (if application made on letterhead)
  4. Legal guardian with guardianship papers

Application for a Florida Birth Certificate MUST Include the Following:

  1. Full name of the registrant as it appears on the certificate
  2. Date of birth
  3. City or county of birth
  4. Names of both parents (including mother's maiden name)
  5. Signature of applicant
  6. Relationship to registrant
  7. Mailing address
  8. A copy of a valid photo identification (front and back)
  9. The fee ($15.00 for 1 copy; $10.00 for each additional copy).

Amendments to a Florida Birth Certificate:

If there is an error on your or your child's Florida birth certificate, contact the State Office of Vital Statistics in Jacksonville, Florida at 904-359-6900, Extension: 9005. Amendment forms are available on the Florida Health Certificates and Registries web page or can be picked up at DOH-Gadsden. This form must be notarized.
